Kait Gillis, Pennsylvania Department of Human Services spokeswoman, said it was too early to comment on pending changes to Medicaid eligibility. The department will continue to work hard to eliminate the challenges, disparities and inequities that still exist as barriers to health care, she said. Expect Medicaid expansion money and subsidies to buy coverage to continue for about two more years, said John McDonough, professor, practice of public health at the Harvard T.H. Chan School of Public Health in Ann Arbor, Mich. After that, a substantial degradation of ACAs provisions is likely. The technical problems on the exchanges could be fixed with ease if there were the political will to do so, Mr. McDonough said. But the Republicans dont want to fix, they want to abolish. Also on the chopping block is the Center for Medicare and Medicaid Innovation, which was created by the ACA to find ways of paying doctors for quality of care rendered rather than number of procedures performed. One center initiative: In April, Pittsburgh-area surgeons treating Medicare patients began receiving a single bundled payment for joint replacement surgery, forcing doctors to find the most efficient, lowest-cost way of getting people better while reducing variability in the quality of care for the 400,000 operations done annually and at a cost of $7 billion for hospitalizations alone. They have to find a way to continue that process, said Dr. Melani, who founded OHara Township-based private equity fund Velocity Fund Partners after leaving Highmark in 2012.
